Social Work & Case Management

To ensure safe and appropriate discharges to our patients, our Case Management experts monitor patient progress and provide appropriate patient support and resources which include:

• Continued care once discharged from GRMC hospital – may be continuum of care at home, skilled nursing facility or an assisted living center, inpatient rehabilitation or a long term acute care hospital.
• Resources and referrals to aid in financial, medical, emotional and prescription assistance.
